The inspection visit took place on December 2 & 9, 2020. This was an unannounced focused inspection in response to concerns raised through enquiries and serious incident reporting about medical care services in relation to: discharge processes and communication; venous thromboembolism assessment and management; incident reporting and sharing of learning including Never Events; support, care and treatment for patients with learning difficulties; staffing; patient care and emotional support; concerns around ‘do not attempt cardiopulmonary resuscitation paperwork; concerns around staff culture; infection control procedures; and nutrition and hydration. Key questions were only re-rated where a breach of regulation was identified.
