The inspection visit took place on December 2, 2020. This was an unannounced focused inspection in response to concerns raised through enquiries and serious incident reporting about medical care services in relation to: discharge processes and communication; venous thromboembolism assessment and management; incident reporting and sharing of learning including Never Events; support, care and treatment for patients with learning difficulties; staffing; poor culture; infection control procedures; and nutrition and hydration. Key questions were only re-rated where a breach of regulation was identified.
